FORREST , MRS. PHYLLIS E. JOHNSON Mrs. Phyllis E. Johnson Forrest, died Saturday, August 20, 2005. Wife of James A. Forrest. SURVIVORS Husband, James; children, Kerry Ed Barr, Theresa Benoit, Fred Janice Forrest, Vern Dena Forrest, Joseph Tammy Forrest; many grandchildren and great grandchildren. Cremation has taken place and at her request, there will be no service. Memorials: The Kidney Foundation or Hospice of N. Ottawa Community.
